How to Apply

Age 21 years and older
Documented cognitive/developmental disability
Eligible for DDS funded day services (Individual Day or Day Service Option) or ability for Private Pay
Self or Guardian must sign all required consent and releases to be able to receive services
Have a required DDS Level of Need (LON) score of 3 or higher or approval for DDS DSO Services if using DDS funding
Physical examination current within one year
Not motivated by vocational / employment services
Emotionally and behaviorally stable. Not requiring nursing/medication dispensing during day or behavioral services and supports
Must have own transport at current time
Private pay per individual to supplement DDS funding
Interdisciplinary team consensus and recommendation. Evaluation by our professionals who strive to make applying both comprehensive and easy
